Terms and Conditions

Waiver of liability & release agreement

I,(“Participant”), agree in consideration of my participation in Chicago School of Golf Inc. (“Company”) programs, I hereby forever release and covenant not-to-sue Company or any of their employees, independent contractors, instructors, volunteers, agents, coaches, and all others who are involved (“Parties”), from any and all present and future claims for property damage, personal injury, or wrongful death, arising as a result of my engaging or receiving instruction in any activities or any activities incidental thereto, wherever, whenever, or however they may occur. I hereby voluntarily waive any and all claims both present and future made by me, my family, estate, heirs, or assigns, and I relinquish on behalf of myself, spouse, heirs, estate and assigns the right to recover for injury or death.

I accept all responsibilities for my participation in Company programs and understand my participation and use of any equipment is at my own risk. I understand that my participation may involve great risk to me, which may include, without limitation, injuries or medical disorders such as heart attacks, strokes, heat stress, strains, sprains, fractures and even death. I have no such physical nor suffer from any medical conditions which would put me at such risk. I assume full responsibility for any injuries, medical disorders or damages which may occur to me or damages to or loss of my property during my participation. Company has made no representations or warranties to me regarding the facility or the equipment or the use thereof. I acknowledge that I will not misuse any of the equipment, facility, or engage in any activity which may put others at risk. I also assume full responsibility for any loss of or damage resulting from my participation.

I hereby exonerate the Company for, and agree to indemnify, defend and hold Company, and Parties, harmless from and against, any and all claims, demands, charges, expenses, including but not limited to attorneys’ fees, damages, actions, and causes of action, whether at law or in equity, now or hereafter existing, and howsoever arising, and in connection with my participation. I understand and acknowledge that this waiver is intended to be as broad and inclusive as permitted by the laws of the State of Illinois, and I agree that if any portion is held invalid, the remainder of the waiver will continue in full legal force and effect. I further agree to be bound to the State of Illinois for any and all legal proceedings.

I affirm that I am of legal age and am freely signing this agreement.

Late/Cancellation – Every new student to Chicago School of Golf will receive a late/cancellation policy contract. This contract will state our very strict policy on cancellation. If you fail to contact your instructor about a cancellation you will be charged the fee stated in the contract.

Student Etiquette – Students are expected to behave and act accordingly on the golf course. This means that all student’s will dress in golf attire for every lesson (See Golf Attire Section) and hold themselves in a way that reflects well on Chicago School of Golf. Use of cellphones or other devices during a private or group lesson is prohibited. It is up to the instructor to cancel a lesson based on the behavior of a student without warning. If the lesson is canceled or the student is dismissed there will be no refund.

Parent Etiquette – At Chicago School of Golf we ask that parents are involved with their junior golfers outside of the lesson. It is very important for parents to push their junior golfers to practice and enjoy the game. However, we ask that for distraction purposes parents do not attend or observe the lesson. Parents may attend the last five minutes of a lesson to hear what their child is working on, so they are informed on how the student should practice. We also encourage parents to call the coach to discuss their child’s game.

Private Lesson Cancellation Policy Agreement:

Chicago School of Golf is committed to providing all of our students with exceptional service. When a student cancels without giving enough notice, they prevent another student from being seen.

Please call us at (708) 816-4653 by 2:00 p.m. two days prior to your scheduled appointment to notify us of any changes or cancellations. To cancel a Monday appointment, please call our office by 2:00 p.m. on Friday. If prior notification is not given, you will be charged full price for the missed appointment.

Camp, Group & Junior Program Refund Policy:

A 20% non-refundable and non-transferrable administration fee for each class applies to all students registered per class.

15+ Days Prior 80%

10-14 Days Prior 40%

7-10 Days Prior 20%

Within 7 days of program start date, No refund

I have read this form and fully understand and acknowledge that, by signing and/or checking the box upon checkout this form, I am giving up/releasing legal rights and/or remedies which may be available to me.
